A. First of all, before even playing the piece, look at chord progressions. Try to find patterns. Also look for phrases or even entire sections that are repeated throughout the piece. As a classical pianist, I tend to feel overwhelmed trying to tackle a large piece ... just notes, notes, and more notes. If you can analyze the piece and find find some sort of pattern (even in small sections), it really helps. Then break it up systematically into sections to practice.
Next, slow practice one hand at a time without pedal, so you can hear every mistake and fix it. Fingerings may sometimes feel awkward at first, but are often necessary in the long run.
For arpeggios and runs, you can try blocking them into chords. If both hands are playing arpeggios, you can play right hand staccato, left hand smoothly. Then alternate back and forth. This improves timing and clarity. Staggering both hands (assuming both are playing the arpeggio) also greatly helps timing (staggering basically gives it a "swing" feel).
In summary, follow these basic guidlines:
-analyze chords and patterns
-slow practice, hands separate, paying attention to fingerings
-focus in on problem spots using various techniques in last paragraph
-slowly increase speed using metronome

A. Piano-you have to learn both the Treble Cleft and the Bass Cleft.
Guitar- you have to learn all the fingerings and chords.

A. Just start reading music. Try to read something new every day. Practicing the same pieces over and over doesn't really help you develop your reading abilities, because you just end up memorizing the piece.
Sight reading skills are basically you memorizing what certain sounds/chords/notes/fingerings look like on paper. It won't take you long before you can spot an open C major chord right away. Over time you'll be able to quickly recognize other chord types readily. It's just a matter of practice.
