A. You could call it a few different things. You really more voicing out a chord. With the notes you are playing you could call it an F9 chord though the 3rd and 7th are missing. It could also be an F sus2. Or you could look at it as stacking 4ths and 5ths. Aaron Copland was famous for using this technique.

A. If you play the notes on a piano, the A and G clash. But it's not bad, it's just a G2 major chord. Or a G9. (A is the second note and ninth note of the G major scale). Btw, the notes in the chord A, D and G make a first inversion G2 major chord.
(if it helps, G-A-D is an example of the root position, A-D-G is an example of a first inversion position, D-G-A is an example of a 2nd inversion position and moving the D back to top brings you back to the root position. It's easier to hear if you're playing a normal triad; like C major or G major.)

A. First, do not strain yourself to reach that high in one day. That will damage your vocal chords, and you will no longer be able to hit even your lower falsetto range. Second, I would try different excersises. Get a piano app for your phone or tablet and sing along to it, each day working higher and higher. Just practice, and don't get stressed if you don't make it there the first day, or the first week, your body is your body and will take some adjusting.
