A. I could give you the notes in the chords, but you may want to use a different voicing. Is this for a college class?
An Am9/G means that it is an Am9 chord with a G in the bass. Am9 has A, C, E, G and B in it.
Am9b5/G would be the same chord as above with an Eb instead of an E.
I could go through them all, but it would take a while.

I've only been able to find one set of tabs for this song, and it's just the intro. They're accurate, but they're not enough, since I'd like to know the whole song.
I'd like to know what the chords are being played during the chorus ("I saw you outside ... I'd like you anywhere, but Black Leaf Falls") and the last two chords played alongside the piano at the end.
If you're doubly talented, I'd also be extremely grateful if you could transcribe the notes being played on the piano at the 2:02-2:15 marks and at the end of the song.
Please note that I have trouble reading sheet music, especially for guitars. For reading guitar music, I prefer tabs. I usually learn piano by watching people play, but writing down the letter notes works, too.
