Q. Ive made a chord sequence on piano and want to play it on guitar but the chords dont sound the same on guitar?
A. There's no difference between chords on the piano and guitar... an A major chord is an A major chord regardless of the instrument you play it on.
The difference is in the chord voicings (the specific order of the notes). Some piano chord voicings are impossible to play on the guitar, and vice versa. That's what makes each instrument unique.
Just look at the voicing on the piano, figure out a way to play it on guitar. If you're playing Bb7 and the notes go Bb F Bb D A, then you just to find a Bb on the guitar and build from there.
If you don't know which notes are which on the guitar, then it's time to learn.

Q. . i play the piano i have natural talent at improvising and have a very good ear. but when i play jazz music i don't know how to incorporate chords and change the voicings.all i play is the root chords so how can i become better? what books are will help me"? or should i get a jazz teacher?
A. get a teacher. books will only get you so far. a teacher can really rocket you to success. I also have an amazing ear and natural talent for improvising on saxophone, piano, and guitar, because of lessons I have taken/ am taking. because of what Ive learned from my teacher, my soloing sound so much better, and even got me a solo with a pro saxophonist from a college near my school! If you get a teacher, and stay committed to learning, i guarantee you will be successful with your piano playing.
How to play the piano using music sheets?
Q. This may be a dumb question, but im quite confused.
I know the notes, and everything about the music sheet, but i dont know
if we are supposed to play ex. "A" chord for the "A" note which is on the music sheet.
Do we play the A Bar on the piano or the whole A Chord for the "A" note on the music sheet?
A. no, if they write the note A, all they want is the note A, and it needs to be in the octave it is written in, if they want the whole chord, they will write all the notes in a vertical stack.
if there are chord symbols above the staff, then you would interpret them as chords, but the notes on the staff are very specific, and let the composer show the exact voicing they might want for a chord.
the chord symbols are used in "lead sheet notation" to give the performer more of an option to play their own choice of voicings.
